The management of intracerebral hemorrhage can be medical or surgical depending upon the hematoma's localization, number, volume, öccurence rate, pathophysiology and other associated factors. There is the ischemic area around the hematoma, called penumbra. The main strategy of the management is to minimize the destrictive effect of the hematoma. Therefore it is necessary to provide the conditions leading recirculation. Surgical evaluation can be advantageous randering early compression and minimizing the neurotoxic effects of hemoglobin degradation products. In this article, medical and surgical management criteria as well as the principles of medical management are discussed. Management of raised intracranial pressure and antiedema treatment choices are overviewed and the debated antiedema effect of corticosteroids is outlined again.
