Purpose: Primary cerebral hydatid cysts are very rare entities. Positivity of indirect hemagglutination and latex tests supports the diagnosis. In this case report we presented a patient (34 year-old, male) whose serologic tests were negative for hydatid disease but has presented with headache, vomiting, dysphasia, right henüparesis and a cystic lesion (47x49 mm) in CT resembling a hydatid cyst. At operation, the lesion was proven to be a solitary hydatid cyst and was removed intactly.

Conclusion: Primary cerebral hydatid cysts may present with negative serologic tests.
