We studied a 12 year-old-man with congenital mirror movement by means of various neurophysiological techniques, including electromyographic studies, movement related cortical potentials (MRCP), somatosensory evoked potentials (SEP) and motor evoked potentials (MEP) to transcranial magnetic stimulation. On EMG examination, we recorded bilaterally simultane response from both extensor digitorum communis muscle. We found bilaterally fairly symmetric latencies and amplitudes on the MRCP and SEP responses. On MEP examination, we obtained similar amplitude responses on both abductor digiti minimi (ADM) muscles with right cortical stimulation; but higher amplitudes on the left ADM with left cortical stimulation. We concluded that mirror movements in our patients might be depending on abnormal organization of the left motor cortex or corticospinal pathways.
