Abulia is not widely used term. References in the literature are limited. Abulia has been retained to refer to the special states of psychomotor retardation that characterise incomplete or partial forms of akinetic mutism. The principal features of abulia are decreased spontaneity and initiative, slowness and paucity of movement, delayed responses or absence of responses, flat affect and indifference. Abulia is generally associated with bilateral lesions of the centromedial core of the brain from the anteromedial frontal lobes down to the upper brain stern. The list of conditions that can cause abulia that the damage is usually structural. Metabolic disturbances and adverse drug reactions can simulate abulia. The relations of catatonic depression to abulia and akinetic mutism is of great theoritical importance. We still face the clinicopathological challenge and opportunity to further explore the anatomic and biochemical basis of abulia
