REM sleep behavior disorder (DBD) is a new type of parasomnia characterized by a history of vivid dreams and excessive movements during REM sleep, sometimes violent ones that cause injury to the patient or bed partner. Polysomnographically, RBD is marked by the absence of muscle atonia; there may even be plasic augmentation of muscle tone during REM stage. Although it may occur at any age including childhood, it is far more common in elderly persons. Various organic brain diseases may underly the disease. When correctly diagnosed, it can be managed satisfactorily, especially with clozuzepam. We studied a 70-year-old man with a 1.5 year history of nocturnal behaviors such as talking, yelling, kicking or punching, and diagnosed as RBD according to his polysomnographic investigation. His MR1 findings were also discussed.
