Neuroleptic Malign Syndrome, an idiosyncratic reaction, is characterized by muscular rigidity, high body temperature, autonomic dysfunction and disturbances of consciousness. Although it is known that this syndrome is seen as a result of neuroleptic medication, it is also proposed that it may follow the use of some other psychotropic drugs and the cessation of previously used dopamine agonists. According to literature reports, its prevalence rate is 0.4-1.4 % and mortality rate is 10-30 %, the causes of which are acute renal failure, myocardial infarction, thromboembolia, respiratory and hepatic failure and sepsis resulted from rhabdomyolysis. It is suggested that some drugs as dantrolene, bromocriptine, diazepam and amantadine are helpful in treating this syndrome. The cases who will be presented here are two examples of neuroleptic malign syndrome treated with ECT while they are resistant to above mentioned medication
